The Patient Flow Center manages the transfer of patients into Vanderbilt University Hospital and all bed capacity of the adult enterprise. This includes interfacing with the emergency department and inpatient units to expedite flow of patients through the Vanderbilt system. The Patient Flow Center is comprised of the following departments: Administrative Coordinators, Admission Coordinators, Bed Management, Decedent Affairs, Patient Transport, and the VUH Transfer Center
JOB SUMMARY: Coordinate efficient and timely placement of patients based on specific medical condition, patient care unit criteria, and bed availability. Communicate admission and other pertinent information with patient care unit(s) staff, faculty, and other facilities staff and physicians.
Assign all beds (surgery, medicine, pediatric) within the hospital in collaboration with the patient care units.
Make decision based on each patient care unit criteria and the availability of beds
Plan for placement early in the day
Admit patients (Medipac) to appropriate status, making changes as needed
Accept reservation on all day of admission into the hospital
Ask appropriate question about patient demographics and medical condition
Quickly and completely communicate this information to admitting staff to facilitate the admission process and to the medical housestaff teams
Maintain accurate assignment sheets
Assigns patients to medical housestaff appropriately and relay admission information in timely manner
Ensure the hospital has required bed available (intensive care and general care) to remain open and avoid diversion of patients from emergency department and/or hospital
Manage the census and bed availability
Plan for each day's admission
Inform the Hospital Administrator on-call when bed availability falls below the requirement so diversion can be evaluated
Communicate clearly and concisely current bed status and outlook
Arrange and prioritize transfer of patients between units and outside hospitals
Management of patient flow
Make placement utilizing all factors and information at disposal
Provide administrative support to department as necessary
Process requests to Release valuables from safe according to policy
Process patient information calls as needed
Field and process patient information calls appropriately making sure call taken care of in timely manner
Process bus, cab and gas vouchers according to policy
Provide appropriate documentation as required
Maintain statistics/information for areas in the hospital for reports as needed
