The Network Operations Director will work in collaboration with the leadership of VHAN and the Connected Care ACO board of director's and its providers to promote clinical integration within the network in achieving the mission, goals and objectives of VHAN commercial and MSSP ACO quality programs.
DUTIES AND RESPONSIBILITIES
Ensure all practices receive practice performance data and understand how to utilize such data to drive practice and clinical transformation
Works with the VHAN medical and executive leadership and the regional leadership to develop and implement a strategic plan for improving care and patient outcomes
Collaborate with the practice leadership, regional medical director, VHAN medical director, and VHAN QI/PI teams to identify opportunities for performance improvement, provide education on recognized best practices and evidence based clinical guidelines, and facilitate program development to achieve goals and objectives.
Collaborate with leadership in developing regular meeting schedule with the affiliate hospitals to ensure effective communication in aligning the mission of the region with initiatives including the execution and tracking of regional goals.
Collaborate with leadership on the recruitment of new practices to the network and ensure a standardized onboarding process to include introduction into the mission of VHAN, physician engagement, quality clinical program goals and objectives, and measures of performance improvement.
Identify and work directly with practices to promote physician engagement with the goals and objectives of network including provider performance improvement, physician attendance to scheduled regional meetings, and adherence to the policies and procedures of the network.
Collaborate with leadership and network providers to identify opportunities for quality and utilization metrics that are aligned with network strategy.
Collaborate with the leadership, regional medical director, practice physician champions, and regional hospital leadership to identify opportunities to improve the quality and cost of care delivered to the network patient population including improving access to care, reducing avoidable admissions and ED visits, management of the complex, chronic care patient, and pharmacy utilization.
Collaborates with the population health analytics team to generate necessary data to support the goals and objectives of the network, promote physician engagement, and measure improvement.
Participate in building and maintaining the physician network
Collaborate with multi-disciplinary clinical operations team including but not limited to care coordination, pharmacy, LCSW to ensure success of clinical and quality initiatives within the network.
Facilitate and co-lead the monthly physician meetings to ensure physician engagement and compliance of network goals and objectives.
Attend and participate in the regional physician meetings and physician leadership council meetings led by the medical director of VHAN, VHAN executives, and/or system leaders.
Collaborates with the managed care contracting teams and attends operational meetings as deemed necessary
Monitors all quality, cost, and efficiency for all contracts on a regular basis
Provides operational insight at all MSSP and Commercial ACO meetings and communicates pertinent information to regional and VHAN leadership
Serves as program manager for strategic initiatives for the MSSP ACO as determined by the ACO Board
Collaborates with the provider offices to improve compliance with the MSSP and Commercial ACO's quality initiatives
