Fremont Hospital is a fully accredited, 148-bed acute psychiatric hospital, providing quality care and assistance for children, adolescents, and adults. Our full spectrum of care includes 24-hour, no-cost assessments, inpatient psychiatric hospitalization, partial-hospitalization, intensive outpatient programs, and a chemical dependency intensive outpatient program. Fremont Hospital is currently seeking an experienced behavioral health professional to join our team of compassionate, dedicated professionals to provide administrative and management functions to ensure the highest quality of patient care.  The position is a Full-Time Adolescent Therapist (32 - 40 hours per week, Tuesday - Saturday or Sunday - Thursday). MFT, MFTi, LSCW, ACSW, LPCC or MSW

POSITION SUMMARY:

This position will perform clinical services functions by providing ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of the individual patient needs. The overall goal of the position is to enhance the quality of management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review, and management of discharge planning. Has accountability for the care, coordination and discharge planning of all patients on assigned unit(s).

The responsibilities of this position include, but are not limited to: group psychotherapy, psychosocial assessment, discharge planning, treatment planning, suicide/self-harm assessment and reassessment, maintaining patient attendance to prescribed treatment, following-up to help ensure the safety of all patients, and ensuring documentation compliance. The therapist will conduct evidenced based CBT/DBT process, psychoeducational, and activity groups. The therapist will conduct brief individual and family meetings.

In addition, the Therapist will be responsible for coordinating the integration of the social services function into patient care, discharge, and home planning processes, interface with Fremont Hospital Clinical Staff, with outside referral agencies, with managed care organizations to ensure that the admission, continued stay, and discharge planning is in compliance with The Joint Commission (TJC), Center for Medicaid & Medicare Services (CMS), and established Discharge Planning Policies and Procedures. Provide oversight and coordination of legal paperwork and Lanterman Petris Short (LPS) regulations and conduct Local Coverage Determination (LCD) Audit as required.
