Communicates client/patient/family healthcare needs to the appropriate health care provider
Provides coordination of client/patient transitions along the continuum of care with appropriate follow-up to provide continuity
Determines appropriate intervention strategies and provides counseling
Access appropriate available resources to meet the identified need of the cline/patient and family
Participates as a member of the care team to achieve business goal, clinical/quality outcomes and customer satisfaction
Responsible for own practice within legal, ethical and professional boundaries
