Works under the supervision of the Special Needs Coordinator and Director, Health Services, Health Choices.
The Special Needs Unit is a regionally based resource to the embedded and community based case managers, Health Managers, members, community agencies, and other health plan staff to address complex member needs.
Serves in an expanded clinical role to collaborate with members, Primary Care Providers, community agencies, behavioral health, dental and other health plan employees to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services within the Primary Care setting. Assess, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the members physical and psychosocial health status.
The Social Worker works collaboratively with the RN Case Manager and care team members to ensure patient needs are met and care delivery is coordinated across the continuum. The expertise of the Social Worker is sought to resolve psychosocial patient care issues and to develop and implement a complex member care plan. This professional ensures that patients are assisted to achieve their highest level of function.

EDUCATION AND/OR EXPERIENCE:
Master's Degree in Social Work (MSW) from accredited university required.
Social Work License preferred (LSW), required within three years of position acceptance.
Two years of clinical experience in social work preferred.
Knowledge of Social Work theories, therapies and techniques as used in individual family and group treatment.
Positions may require adult and/or pediatric specific experience.
Knowledge of the basic concepts and principles of managed care preferred.
Critical thinking skills required. General computer knowledge and capability to use computers required.
