Initiates and maintains all patient medical records, discharge records, and outpatient records. Assembles and analyses all medical records for completeness. Locates and eliminates duplicate medical records, both physically and electronically.
EDUCATION: High school diploma required.
CERTIFICATION/LICENSES: RHIT and Notary Public Certificate preferred.
SKILLS: Basic personal computer and keyboarding skills. Medical terminology, JCAHO standards familiarity, phone etiquette, and good spelling abilities preferred.
EXPERIENCE: Two years of clerical work experience required, including a minimum of six months' experience in a healthcare setting such as a clinic or hospital, including familiarity with medical records.

PHYSICAL REQUIREMENTS: Moderate physical effort (able to lift/carry up to 25 lbs.). Frequent prolonged standing, walking, sitting. Occasional lifting of supplies/equipment. Occasional reaching, stretching, bending, kneeling. Data entry keyboarding up to five hours per day.
