The Care Coordinator delivers innovative, proactive and compassionate service that exceeds our member's expectations. The Care Coordinator is the knowledgeable and trusted advocate for Prominence members, as well as represents facility and its policies and procedures when assisting customers through initial contacts, education opportunities, inquiries, problems or complaints. The Care Coordinator provides effective two-way communication to ensure quality services and appropriate problem resolution for all parties involved. The main focus of this position is contacting Prominence Health Plan Medicare members by phone to inform and educate them on the health insurance offerings, products or services, as well as respond to inquiries and assist with quality improvement and member health assessments. The Care Coordinator will work with Senior Care Coordinators, other Prominence Department members and health care providers, on helping members develop realistic and attainable mental and/or physical health goals, and assist members in identifying needed resources to help accomplish member goals.
