Resolves ACO quality and operational issues such determining performance, quality, and transition of care. Ensures all clinical operations comply with ACO guidelines and analyze data to improve the quality of care delivered. Maintains educational information databases for physicians and contracting party. Reports to Director of Operations.
JOB SPECIFIC DUTIES
Performs pre-visits for review of health plan's patient medical record to determine gaps in quality measures. Conducts patient chart review in office if deemed necessary. Prepares visit checklist to ensure accuracy of quality measures due to be completed on time. Assists in a clinic service and collects relevant patient surveys and if applicable, reviews of patient medical record for past visits in calendar year. Leaves a copy of records indicating services and results provided by ACO outcome guidelines. Provides PVD, Neuropathy and COPD screenings. Schedules follow up appointment with assigned PCP to ensure all results from the ACO guidelines are reviewed with the patient. Conducts patient home visits as needed Travels between facilities as requested Other duties as assigned by management
