Provides proactive case coordination for patients, including discharge planning, utilization management and resource management. Organizes services across the continuum from pre-registration through discharge to affect optimal patient outcomes, achieve continuity and quality of care, reduces costs and provides patient satisfaction. Provides assistance to patients and families through evaluation of social, emotional and financial needs and coordinates and facilitates appropriate resources. Communicates with third party payers. Follows UM policies and procedures, and assists with continually improving the quality and effectiveness of case management at St. VincentRegionalMedicalCenter.
Graduate of accredited college/university in Nursing, BSN preferred or a Masters degree in Social Work, (MSW or MSSW).
Current licensure as an RN or MSW in New Mexico; BLS recommended.
Excellent reading and writing skills in English. Strong organizational skills. Excellent interviewing skills and ability to assess patient needs. Ability to remain calm in high stress situations. Ability to effectively evaluate medical records. Competent computer skills for word processing, data collection and retrieval.
Minimum of five years of experience in a health care setting preferred, of which a minimum of two years has been spent in an acute care setting. In depth knowledge of utilization management criteria, complex medical and surgical patients, reimbursement and regulatory requirements including, HCFA guidelines.
NATURE OF SUPERVISION:
-Responsible to: Director, Inpatient Rehab Services
General office environment and general hospital department/nursing units (100% of day).
Blood borne Pathogens: A
Ability to move around the hospital to all units/department for about 80% of the day, in office 20% of the day. Requires close work, good vision, dexterity to write as well as to use computer keyboard.
