JOB SUMMARY: The Discharge Planning Coordinator provides discharge planning services in a manner consistent with the philosophy and objectives of the facility.
To perform this job successfully, an individual must be able to demonstrate competency in the criteria listed in this job description.Â The specifications listed below are representative of the knowledge, skill and ability to meet the minimum requirements for this position.Â Staff must demonstrate flexibility for reassignment, providing care to varied populations including male and female children, adolescents and adults as well as dually diagnosed adults.
Education:Â LPC, LPC-I, LMSW, LCSW preferred, or Bachelorâ€™s degree in Mental Health field with a minimum of two yearsâ€™ experience
Experience:Â Â Minimum of 2 years clinical experience
Knowledge:Â Â Â Knowledge of case management; discharge planning, age specific growth / development, crisis / behavior management / community resources
Supervision Received:Â Â Â Â Â Â Â Â Â Â Â Director of Clinical Services
In absence ofÂ qualifications upon hire, must demonstrate ability to learnÂ concepts / developÂ skills within reasonable time frame via the organizationâ€™s orientation and training program.
Exposure to psychiatric patients who may exhibit violent/aggressive behavior.Â Potential for exposure to communicable diseases, blood/body fluids and other hazardous waste.Â General hospital environment.
