As part of the Telecare Family, the San Bernardino Forensic Assertive Community Treatment (FACT) is an Assertive Community Treatment (ACT) program and serves 50 unduplicated clients, ages 18+, that may have a history of frequent psychiatric hospitalizations and a history of homelessness.
POSITION OBJECTIVE • Using Psychiatric Rehabilitation principals and Recovery Philosophy, the Team Leader provides clinical and administrative supervision, coaching and education for assigned team members with appropriate psychiatric support to ensure service excellence and courteous, helpful, and respectful services to program members. • Provides direct member, community support and case management services to assigned members who are involved in the justice system. • Provides education and support to members and their families • Coordinates service needs and collaborates with other services and agencies such as the Conservator's Office, Public Defender's Office, Child Protective Services, Probation Department, California Department of Corrections and Rehabilitation, and Mental Health Authority • Completes assessments and determines the appropriate level of services for new members
?QUALIFICATIONS • Mental Health professional: MSW, MA, MFTI, RN, Registered Psychologist- Licensed Waivered; or (current and valid LCSW, LMFT, MFCC or LPC) • Outpatient clinic or community mental health service with two years of direct clinical service is preferred • One year supervisory experience with strong leadership skills is also preferred • Experience in providing intensive case management, intensive community supports using psychiatric rehabilitation concepts is highly desirable • Sensitivity to multi-cultural populations and issues is required • Basic computer skills are required • Valid California driver's license, a MVR clearance for insurability, a vehicle (approved for safety), and proof of insurance is required • Applicant must be fingerprinted and receive clearance from the Department of Justice and the Office of Inspector General
