Saint Simons By-The-Sea Hospital is a 101 bed psychiatric and addictive disease hospital for adults and adolescents. Saint Simons By-The-Sea Hospital is seeking a Health Information Management Clerk, who is responsible for rendering to Administration any assistance in contributing to the continuous efforts to elevate the standards of patient care and documentation and evaluation thereof.Â Analyze, organize, file and maintain current and archived patient charts, as well as maintain and respond to all departmental correspondence.Â
Responsibilities include, but are not limited to, the following tasks:Â
Assemble and analyze discharged patient records.
Complete daily â€œRelease of Informationâ€� forms of discharged patients and fax information to appropriate aftercare provider within 5 days of discharge.
Maintain a log of aftercare ROIâ€™s that were not faxed as a result of being incomplete.Report log to HIM Manager on a monthly basis.
Pull charts for Core Measures, Peer Review, Utilization Review, Risk Management, and others, as requested in compliance with department procedures.
Maintain a log of Outcome Core Measure Basic 32
Assist Medical Staff in identifying chart deficiencies and signing of incomplete medical charts.
Work with co-workers in purging charts from previous year to prepare for offsite storage.
Answer telephone calls from patients, aftercare providers, and others in a timely and courteous manner.
Assure that medical records are maintained as secure and in the best interest of the hospital.
Follow Saint Simons By-The-Sea Hospital Time and Attendance policy.
Will adhere to all hospital policies and procedures.
Support facility internal and external customer service standards.
Comply with practices and procedures of Local, State, & Federal regulatory agencies for appropriate usage and maintenance of patient confidentiality.
Assist HIMÂ Director, as needed.
