The Director of Assessment/Referrals (Intake) directs and manages the day-to-day operation of the Intake Department, ensuring prospective service recipientsâ€™ linkage to appropriate resources or care providers responsive toÂ individual needs and supportive ofÂ the organizationâ€™s overall mission, philosophy, and goals.
Â Responsibilities include:
Provides Needs Assessment department coverage; conducts screening and/or assessments, in keeping with clinical knowledge and skill level, as necessary to efficient operation of dept.
Facilitates clinically appropriate admissions to organization services, congruent with needs and resources of caller
Makes effective referral of callers to resources that optimally meet callerâ€™s needs; demonstrates sufficient understating of services available in community; is consistently available for staff back-up during high volume times.
Prepares and submits timely, complete, and accurate daily, weekly, and month-end reports
Interacts effectively with physicians and other clinical staff, supporting philosophy, and implementation of programs
Interacts well with referral sources: facilitates prompt, effective communications with these individuals and/or services, in interest of patientâ€™s and organizationâ€™s operations.
Carries out the personnel management activities associated with direct staff supervision, including screening and selection, orientation training and development, performance management, and employee relations functions
