Care Coordination Responsibilities:-� Interpret clinical data to make recommendations on discharge disposition or validate existing recommendations (�Next Site of Care�)-� Communicate with hospital care teams on expectations for discharge disposition-� Communicate with post-acute care teams on expectations for post-acute care pathways-� Discuss deviations from plans of care or care expectations-� Facilitate warm hand-offs and transitions between different levels and sites of care-� Conduct site visit regularly to the next site of care to ensure continuity of care cross care delivery continuum-� Function as care management liaison among different care delivery entities --Operational Responsibilities:-� Collaborate across the organization with various members of care teams including physicians, case management, physical therapy, ACO/Population Health, and clinical service line leaders-� Support recurring operational analyses of BPCI-A program performance and hospital based performance to validate existing operational interventions and identify additional BPCI-A and other suitable interventions; collaborate with clinical and operational leaders to support implementation of new BPCI-A program and institutional specific care redesign initiatives in the acute and post-acute settings -� Support training and communication efforts to involve and engage clinical and non-clinical staff in all care redesign initiatives -� Support the organization�s engagement with other healthcare provider organizations involved in BPCI-A program management and care management redesign initiatives, including post-acute care providers, physician groups, and other entities, include care redesign and related contracting requirements. -� Monitor data accuracy in patient tracking platform (�Episode Connect� & PAMS discharge tracking platform) and take proper measures to fill in gaps or escalate.
BSN degree required. Completion of Clinical experience in NLN accredited school preferred. Additional experience required based upon specific department needs. Current NJ RN Licensure required. National Certification preferred. BLS required.
