Works with physicians and multidisciplinary team members to develop a plan of care for each assigned patient from admission through discharge. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. Assesses and responds to patient/family needs by coordinating the efforts of other team members. Identifies and resolves barriers that hinder effective patient care
A graduate of an accredited Registered Nursing program, BSN preferred or
A graduate of Social Work Program, MSW or LCSW preferred
Basic knowledge of computer systems
Good typing skills
Excellent customer service skills
Excellent negotiation skills
Excellent time management skills
Minimum three years diverse clinical experience as a Registered Nurse or Social Worker
Minimum two years case management and/or utilization review experience.
Behavioral Health experience preferred
Transition of Care experience preferred
C. Licenses, Registrations, or Certifications:
Current/Active unencumbered Texas Compact RN Licensure or
Current/Active unencumbered Texas Social Work Licensure
Ability to obtain Louisiana RN or SW licensure.
Certification in Case Management required, or obtained within 6 months of hire.
