Job Summary Accountable for the coordination of care across the continuum for BMT patients and families. Oversees nursing care provided by assistive providers. In coordination with the nurse educators, nursing managers, Medical Director, BMT staff, and interdisciplinary team members, establishes a total care program. Actively communicates pertinent information to those involved in the care of the patient so that well informed decisions are made in a timely manner.
The position is associated with a significant level of autonomy; therefore, the individual must be highly motivated and able to work independently. Demonstrates skill as a leader, researcher, and educator.
Organizes and helps facilitate continued care after consultation based on the plan of care for each patient. Consults with BMT patients/families once an initial appointment is scheduled to ensure all records are available and to describe the spectrum of services available. Actively participates in all patient consultations with the Physician and Nurse Practitioner. Provides ongoing assessment and evaluations of symptoms related to the diagnosis or related to treatment and contributes evidence-based suggestions for symptom management. Optimizes patient care.
Facilitates referrals to interdisciplinary team members; including referral to other specialists, supportive care consults, scheduling diagnostic studies, and follow-up visits.
Assesses patient and family complex needs. Organizes and provides extensive education and support via face-to-face contact, electronic media and phone consultation. Helps them through the diagnostic and care process, providing information for informed decisions, answers any questions or concerns. Provides support and resources for psychological needs in difficult time.
Acts as a liaison to patients, health care providers, support services and/or referring organizations at the local and national level to promote seamless patient care.
In collaboration with the BMT Director, establish and maintains BMT Total Care program. Focuses on evidence-based solutions, process improvement and innovation to increase positive patient outcomes and satisfaction.
Represent the BMT Total Care Program at local, national, and international meetings. Collaborate with affiliated organizations to enhance program quality and resources.
Participate in and monitor the conduct and quality control of clinical trials as assigned.
