Works under the direct supervision of the Regional Manager, Health Management.
Serves in an expanded nursing role to coordinate and provide disease management education and clinical management for Geisinger Health Plan members enrolled in Health Management programs. Collaborates with primary care providers in multiple practice sites to facilitate clinical guideline adherence and coordinate services specifically related to the members health care needs. Integrates identified member needs, education pathways, clinical guidelines and diagnostic tests to develop individualized plans of care.
Utilizes marketing skills in the Health Plans network of providers to identify and enroll members with chronic health conditions into Health Management programs, as well as refer members to appropriate case management and specialty services. Assesses community and employer work-site needs to support wellness and health management programs.
Collects and analyzes data, maintains documentation and adjusts interventions to maximize clinical, quality, and fiscal outcomes. Participates in Quality Improvement activities as required.

EDUCATION AND EXPERIENCE:
Requires a RN license, or eligible for licensure in the state of Pennsylvania. Bachelors degree in nursing and/or health care preferred.
Minimum of three to five years experience in clinical nursing with emphasis on patient education and program management required. Ambulatory care/managed care experience is preferred.
Computer experience required.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Requires the ability to travel to multiple office locations and manage fluctuating work hours, including off shift, weekend, etc. as needed. Valid Pennsylvania driver's license and willingness to travel as required.
